I got some PM (that is my setting) notifications. The problem as I see it, is at the subscription level.
This part of the Forum software, requires sending and receiving mails since the user is about to be subscribed and has to confirm his choice(s) via mail.
This Forum section is driven by another set of PHP scripts and there might be some mismatches.
Although in my case the mails went to SPAM, in the Trinidad and LLuser cases, it seems that the mails never left the server, assuming that they were generated (Were they?).
I have no idea regarding the CRON settings, but when I had to make fine tuning to the amount of mails, recommendation was to stay at most 150, better below.
Maybe this is the reason. A lower setting for the mail burst, might solve this.
Say, 120 mails (or less?) instead of 150.
Now given the fact that this is an old Forum and the amount of mails sent from the server might sky-rocket in some days, some mails might never make it out.
In order to test all this stuff, I usually create one or two dummy accounts, with zero privileges (Newbie subscriber) and I log in like that. 
Assuming all went well and the account was successfully created, I have a good image of all the stuff that goes behind the scenes.
I also test the password reset scripts, in order to avoid unpleasant situations.
This type of approach is very useful especially when I need to change the standard messages sent automatically (Birthday, Subscriber Welcome, etc.)
I kinda hate those, but the goods come in package so it's kinda "Take it, or leave it!" deal. Any in-between, means adding mods and scripts and the story goes on endlessly...
Another thing I stumbled upon over the years, are the "Google bonus features", such as flagging as "Untrustworthy" an entire site, just because it has been refaced or somehow changed (subdomain added or the like). It is impossible to know beforehand what really triggers the Google Tyrannosaurus behind the scenes.
Also, an issue usually is DNS propagation. Theory says "48 hours" but this proves to be very relative.
It largely depends on the local network strength and individual ISP policies.
Romania has a very powerful and fast network overall, but this is rarely the case in many other locations, even in Europe. Let alone other locations such as islands or low demographic areas. Hardware is very expensive, although few people understand this.
Greece, Germany for one, have very high cost rates for fast networks.
My wife works in Germany, has a 5G cube and the speed rarely reaches 10 MBps.
England (UK in fact) also has high prices for fast internet connections. Same speed tested repeatedly by one of my close friends living in a London suburb.

And the most funny thing is that there are so few people that understand one thing: shared connection.
People nowadays believe that the (smart) Phone is "the deal".
I doubt that there are many people that understand that their connection is shared among their devices and this drops down the speed dramatically.
In my case, when My wife is at home, there are 4 phones + 1 laptop + 1 desktop. Assuming that I get the full speed, that would be 490 Mbps/6. Not quite the same thing as the theory. But, still very good.
Most people expect the same speed, regardless the number of devices.
